Q: What number do I call to make an appointment to get an ID card made?
A: Call Customer Service at 623-856-8970

Q:  What is needed to renew an ID?
A:  Military:  expired ID
Civilian:  DD Fm 1172-2 from Human Resources
Dependent:  DD Fm 1172 signed by sponsor if sponsor is not available

Q:  Who can sign a DD Fm 1172 application for dependents?
A:  The sponsor.  If the sponsor is not available to come in, the DD Fm 1172 must be signed and notarized by sponsor or the dependent must have a power of attorney from the military sponsor.

Q:  What if my spouse is deployed?
A:  The dependent may bring a copy of the sponsor’s deployment orders or power of attorney.  The military sponsor can also visit the nearest base at the deployed location to obtain a DD Fm 1172.  The 1172 can be faxed from one military installation to another.  

Q:  How old does the dependent have to be to receive an ID?
A:  10.  (If both parents are active duty or if parent is single and the child resides in the home, dependents under 10 are eligible to receive IDs)

Q:  What about dependents over 21 years of age?
A:   A letter is need from an accredited college registrar’s office stating full time status with anticipated graduation date.  This must be provided until dependent turns 23 years of age.

Q:  What documents are needed for a ward (without intent to adopt) of custody?
A:   Birth certificate and legal decree from a court of law or placement agency.

Q: What documents are needed for pre-adoptive ward?
A:  Birth certificate and legal decree or official documents from DoD recognized placement agency where adoption procedure will be completed.

Q:  What are the requirements for Disabled Veterans?
A:   Must have letter from VA showing 100% and that commissary and base exchange (only) are authorized.
